PURPOSE:

The proposed rule amendments are intended to incorporate the revised application forms which provide for a one-time reduction in licensure fees. Additionally, form DOH-MQA 1072 (Medical Faculty Certificate), has been revised to reflect the name change for Mayo Clinic and the addition of Johns Hopkins.
