PURPOSE:

The proposed rule amendments are intended to incorporate the revised application forms into the rule.

SUMMARY:

The proposed rule amendments incorporate the revised application forms into the rule. Specifically, when the forms were recently amended, the statutorily mandated unlicensed activity fee was inadvertently omitted from the applications. Form DH-MQA 1008 is also being amended to include a question to satisfy Section 458.317(1)(a), F.S., and DH5001-MQA is also being amended to include the Florida Birth Neurological Injury Compensation Association (NICA) form.
