RULE:

64B27-1.002

Dental Laboratory Biennial Registration

PURPOSE:

To incorporate by reference into rule a registration form for dental labs in accordance with legislation passed during the 2009 Session.

SUMMARY:

This rule incorporates an application form including questions required by Section 456.0635(2), Florida Statutes.
